Terms and Conditions
This agreement contains information about your Johnson Bank Visa
Gift Card (the "Card"). In this Agreement, the terms "you" and
"your" mean the person that has received the Card. "We", "us" and
"our" mean Johnson Bank, our successors, affiliates or assigns.
Business days are defined as Monday through Friday, excluding
federal holidays.
Read and review this Agreement carefully and retain it for your
records. Sign your Card immediately. Record your Card number for
future reference. When you purchase, sign, or use your Card, or
allow another person to use your Card, you agree to be bound by
these Terms and Conditions. Go to the Gift Card main page to view the most
recent Gift Card Customer Agreement.
REGISTERING YOUR CARD
In order to help us better serve you it is important that you
register your Gift Card at the Gift Card main page. We will then be
able to access information on your Card in the event your Card is
lost or stolen, if you contact us to resolve a potential error on
your Card, or if you contact us for assistance with your Card. If
your Card is not registered, these services may not be available to
you.
HOW TO USE YOUR JOHNSON BANK GIFT CARD
- Register your Card at the Gift Card main page.
- Your Gift Card is ready to use anywhere Visa is accepted with
certain exceptions:
- Pay at the pump is not available with the Gift Card. If you use
your Card to purchase gasoline, you will need to pay the gas
station attendant.
- Car rentals, hotel rentals and Internet gambling are not
available with your Gift Card.
- Cash Advances are available through any VISA processing Bank or
Credit Union that offer Cash Advance options.
- ATM withdrawals or cash back at a point-of-sale terminal are
not available with your Gift Card. A Personal Identification Number
(PIN) is not issued with the Gift Card nor do you need a PIN to use
the Card.
- You may use your Johnson Bank Gift Card to make online
purchases.
- If your purchase exceeds the amount of funds available on the
Gift Card, you may ask the merchant to split the transaction. You
may pay the remaining amount of the purchase with another form of
payment.
AVAILABLE TRANSACTIONS
Prepaid Card; FDIC Insurance
This Card is a prepaid card access device that can be used at
retail establishments that have agreed to accept Visa-branded debit
cards, with certain exceptions as shown above. Additional value
cannot be loaded to the Card. The Card is our property and we may
suspend, revoke, or terminate it at any time at our sole discretion
without cause or notice. The value of all Cards is held in an
aggregate account that is covered by FDIC insurance.
Card Access
You may use your Card to pay for purchases at retail
establishments that have agreed to accept Visa-branded debit cards
and are equipped with a point-of-sale terminal that can process
such transactions. You may be asked to provide an acceptable form
of identification in order to make a purchase or perform a
transaction using the Card. If you allow someone else to use your
Card, we will consider you as having authorized this person to use
your Card and you will be responsible for any transactions
initiated by such person with your Card. Certain retail
establishments such as restaurants may authorize amounts in excess
of the actual purchase amount to cover tips, gratuities and other
charges. The addition of this percentage may result in a decline of
your Card even though sufficient funds are available. The time
frame for releasing amounts authorized in excess of the final
purchase amount is dependent on the speed at which the merchant
processes the final transaction. If a purchase exceeds the
available balance on your Card, ask the merchant if you may use the
funds on your Card toward a portion of the purchase price and use
another form of payment for the remaining amount of the purchase
price. Use of your Card for any activities that appear to be a
violation of the law may result in the transaction not being
authorized.
Transactions
Using your Card to purchase goods and services from merchants
constitutes a demand against and withdrawal from the remaining
value on your Card. You are not allowed to exceed the balance of
the funds available on your Card. You may use your Card only in the
manner and for the purposes authorized by these Terms and
Conditions. You should receive a receipt for each transaction
completed with your Card. You should retain your receipt(s) for
your records. You do not have the right to stop payment on any
point-of-sale transaction originated by the use of your Card. You
are responsible for all transactions initiated by the use of your
Card.
Foreign Transactions
If you make an international transaction using your Card, Visa
will convert any foreign currency into U.S. dollars using an
exchange rate for the applicable central processing date that is
either: the date that is selected by Visa from the range of rates
available in wholesale currency markets (which rate may vary from
the rate Visa receives) or the government-mandated rate. An
International Service Assessment Fee of 1% on all international
transactions (excluding those from Puerto Rico and the U.S. Virgin
Islands) will be assessed if the transaction involves a currency
conversion. This fee may be passed on to you.
INFORMATION ABOUT YOUR CARD AND RELATED
FEES
It is important for you to track the available balance on your
Card. You can do this by calling 1-800-538-7670 or going to the Gift Card main
page. Except as otherwise required by applicable law, your Card
will expire after the last day of the month shown on the front of
the Card, or when your Card balance reaches zero due to purchases
or fees that have depleted your Card balance. You agree that any
balance remaining on your Card may become subject to applicable
state law. You may request a check for the remaining balance on
your card on the expiration date and up to 3 months following the
expiration date shown on the front of the Card. However, a
check-issuance fee** and all applicable maintenance fees will be
deducted from the remaining balance on the Card. You can request a
check by calling 1-800-538-7670 or by visiting a local branch. If a
check is not requested within 3 months of the expiration date on
the Card, you will forfeit all funds remaining on the Card. Your
Card must be registered with us in order to cancel your Card and to
receive a check.
LOST OR STOLEN CARD*
Tell us AT ONCE if you believe your Card has been lost or
stolen. Calling us at 1-800-538-7670 is the best way to do this. A
replacement Card may be issued minus a reissue fee**. Your Card
must be registered in order for us to cancel your Card and issue a
replacement Card. Any unused funds available on the Card at the
time you contact us will be reissued on a new Card** or in the form
of a check***. The Bank is not responsible for any funds used prior
to your reporting the Card lost or stolen.
DISCLOSURE OF INFORMATION TO THIRD PARTIES
We will disclose information to third parties about your Card or
transactions that you make using your Card only under one or more
of the following circumstances:
- Where it is necessary to complete a transaction;
- In order to verify the existence and condition of your
Card;
- In order to comply with applicable government agency or court
orders or as otherwise required by law;
- For analytical purposes;
- To our employees, auditors, affiliates, service providers, or
attorneys as needed;
- If we conclude that disclosure is necessary to protect you or
the interests of the Bank; or
- If you give us your permission.
ERROR RESOLUTION PROCEDURES*
You may contact us regarding errors or questions about
transactions arising from the use of your Card. Call us at
1-800-538-7670 or write us at Johnson Bank, Gift Card Department,
P.O. Box 787, Kenosha, WI, 53141-0787, as soon as possible if you
think your receipt is wrong, or if you need more information about
a transaction listed on the receipt. Our customer service
representatives are available 24 hours a day, 7 days a week. We
must hear from you no later than 60 days after the date of the
transaction in question. Please provide the following
information:
- Your name and Card number;
- A description of the error or the transaction you are unsure
about;
- An explanation as to why you believe there is an error or why
you need more information; and
- The dollar amount of the suspected error.
If you contact us by telephone we may require that you send your
complaint or question in writing within 5 business days. Generally,
we will determine whether an error occurred within 5 business days
after we hear from you and will correct any error promptly. If we
need more time, however, we may take up to 90 calendar days to
investigate your complaint or question. If we ask you to put your
complaint or question in writing, and we do not receive it within 5
business days, we may decide not to process the dispute. We will
advise you of the results within 3 business days after the
completion of our investigation.

We reserve the right to change these Terms and Conditions, or
add new Terms and Conditions at any time, with or without cause,
and without giving you notice, in accordance with applicable
laws
